Money Follows the Person Program
In 2011, DAIL was awarded a five year $17.9 million “Money Follows the Person” (MFP) grant from the Centers for Medicare and Medicaid Services (CMS). The grant was continued effective April 1, 2016 though September 30, 2019 with an additional $8 million. The goal of the MFP grant is to work together with the Choices for Care Program to help people living in nursing facilities to overcome barriers that have prevented them from moving to their preferred community-based setting.

The Adult Services Division is responsible for the certification of Case Management Services providers and Adult Day Centers.

Whom We Help
The Adult Services Division (ASD) is responsible for the management and oversight of a full array of long-term services and supports for older Vermonters and adults with physical disabilities. The ASD supports Vermonters to live as they choose, pursuing their individual goals and preferences within their chosen communities.
