Social Connection Map
The Age Strong VT Plan seeks to combat social isolation and strengthen social connections for older Vermonters. This mapping tool, developed by a team from the Vermont Certified Public Manager’s Program (VCPM) in collaboration with the Vermont Center for Geographic Information, worked diligently with the Department of Disabilities, Aging and Independent Living Adult Services Division staff to develop a mapping tool.
The intent of this mapping tool is to assist in increasing social connection and engagement for older Vermonters by identifying the network of viable senior centers, adult-day centers, and other local community institutions with quality and equitable programming.
This map will be updated annually to keep resources current and for gaps to be identified to expand resources in underserved communities.
