Moderate Needs Services
Choices for Care provides limited funding for homemaker, adult day, case management, and ‘flexible fund’ services to people in the “Moderate Needs Group”. People in this group do not meet nursing home level of care criteria. The intent is to prevent or delay the need for more costly long-term services and supports by providing these services. People do not need to be Medicaid eligible, however, funding is limited.
