The Adult Services Division is responsible for approving all new providers of the Choices for Care (home-based, Adult Family Care, Enhanced Residential Care) and Traumatic Brain Injury programs. This page provides an overview of the provider enrollment process by provider type and provider-specific fillable enrollment applications.
Choices for Care provides limited funding for homemaker, adult day, case management, and ‘flexible fund’ services to people in the “Moderate Needs Group”. People in this group do not meet nursing home level of care criteria. The intent is to prevent or delay the need for more costly long-term services and supports by providing these services. People do not need to be Medicaid eligible, however, funding is limited.
Family caregivers are critical to the success of seniors and people with disabilities living in the community. The Alzheimer’s Association estimates that in 2012 30,000 Vermont caregivers provided 34 million hours of unpaid care to people with dementia.
