Need Assistance Resolving a Complaint About Services

Do you need help resolving a complaint about the services you receive at home through Choices for Care or in a licensed residential care or nursing facility?

Here are a few ways to get your complaint submitted and obtain assistance toward resolving.

You may contact the Vermont Long-Term Care Ombudsman services for help in resolving your complaint. The Ombudsman is unbiased and works on behalf of the person receiving services.  Contact information: Phone at 1-800-889-2047 or visit their website at Long-Term Care Ombudsman Website.

You may also contact the Adult Services Division at 802-241-0294 or use the Adult Services Division Complaint Form to submit a complaint.  Please do not include any social security numbers, dates of birth, or account information of any type in this form.
