People who are in need of assistance and a place to live, may be eligible for services to help meet that need.
Adult Family Care
Adult Family Care (AFC) is a 24-hour, home-based, shared living option for eligible Choices for Care (CFC) participants. Authorized Agencies (AA) are paid a daily tiered rate by Medicaid to contract with private, unlicensed family homes that serve one to two people that are not related to the home provider. The home provider receives a tax-free stipend to provide services to the person.
Home Share
“DAIL supports an innovative Home Share Program in Vermont: HomeShare Vermont which has been successful in helping older Vermonters and those with disabilities stay in their own homes, as well as in helping people find affordable housing. HomeShare serves Addison, Chittenden, Franklin, Grand Isle, Lamoille, Washington, Orange and most of Windsor County. HomeShare recently began serving select towns in the Northeast Kingdom and Rutland County as well. Homeshare’s case managers arrange live-in ‘matches’ between Vermonters who have a living space to share and others who need an affordable place to live and can offer support (such as housekeeping, yard work and socialization). All participants must pass background and reference checks before beginning the match process.”
Nursing Facilities
Vermont Licensed Nursing Facilities follow all federal and state regulations for providing care. Most accept Medicare and Medicaid coverage for services, in addition to private pay. Nursing facilities provide a high level of care with 24-hour nursing services, including room & board. The Division of Licensing and Protection enforces state and federal statues and regulations for Vermont Nursing Facilities.
Residential Care and Assisted Living Residences
Vermont offers two licensed residential care home options: Level III Residential Care and Assisted Living Residences. Homes provide assistance with personal care, nursing overview and 24-hour oversight. Approved homes may offer Medicaid coverage for services as well as private pay.
