ASD is happy to announce that the Provider Self-Assessment Survey for the new federal Home & Community-Based Services Medicaid (HCBS) regulations is ready for distribution! Providers of Choices for Care Case Management, Adult Family Care and Traumatic Brain Injury (TBI) have been notified that the survey will be distributed April 3rd and is due April 28th. Responses to the surey will help us better understand how our providers' current operations align with the new federal HCBS rules, and how we can work together to make any necessary changes.
